Synopsis of Incident
As the employee was exiting the elevator on the second floor of the Tower Building, she tripped on the "safety cone," which was a few feet from the elevator entrance. The employee fell to the floor, causing injury to her elbows and right hip. Employee failed to report the incident at the time it occurred. she reported the incident approximately two hours later.
