Synopsis:
While employee was walking eastbound on the sidewalk, just passed the north
employee entrance, the employee fell face forward to the ground. Known employee
sustained facial and head injuries. Tempe Fire Department #E272 responded and
provided medical treatment. Employee refused to be conveyed. Employee's family
member responded and transported employee from college.
