Synopsis of Incident

Individuals began to evacuate the building.  It was identified via the Director of Maintenance & Operations, that the system had malfunctioned while the system was placed in "Test" mode, due to the water flow.  The fire department was called off.  The "All Clear" was announced by Director of Maintenance & Operations.

Synopsis of Incident

Known student unintentionally damaged a parked vehicle, while attempted to park her vehicle.  College Safety Officers responded.  Both individuals observed the damage to the parked vehicle.  Both individuals agreed to exchange their personal information to satisfy the damage and repair.  No police summoned.

Synopsis of Incident

Unknown person entered the office of an employee.  While the unknown person was inside of this office, this person took without permission the coffee cup of the employee.  Value: $10.00.  Employee left her office at 9:00 PM, the end of her work day.

Synopsis of Incident

During a telephone conversation known student became upset after receiving information that he was not approved for financial aid.  During the telephone conversation student became extremely agitated, while making verbal threats of reporting to the college.  After being advised of threat being documented, student apologized.

Instructor reported to Administration that between June 8, 2012, to today (Tuesday, June 12, 2012), all textbooks were accounted for.  On Tuesday, June 12, 2012, 5:45 PM, instructor discovered five to ten textbooks were taken without permission from classroom 1075.  Police notified.

Synopsis of Incident

Student complained to instructor of being fearful of two other classroom students.  All three students have been arguing back and forth, due to unacceptable classroom behavior.  College Public Safety was summoned to classroom, at which time, all students were advised of expectations.  No further problems to report.

Synopsis of Incident

While inside of the GED classroom, two known students began to argue with one another.  After being advised to stop their unacceptable behavior, both students continued to be disruptive inside of the classroom.  College Safety Officer responded, at which time one of the students had left the classroom and later left the building and property.

Synopsis of Incident

Student became irate after not having enough time to complete her test, as previously advised.  While student was upset and verbally abusing the employees, the student suffered a possible "panic attack."  Emergency Services were summoned.  Student refused medical attention.  Student's mother became upset as well.  Both left the building and property, after the student refused medical attention.

While employee's vehicle was parked in the parking lot, another vehicle (blue) parked next to employee's vehicle, damaging the left front fender.  The incident was not viewable via the recorded camera footage.  Police was not summoned.  Employee became upset and left the building and property.

Synopsis of Incident

While at the Cafe Breakfast Bar counter, employee opened the pastry bin, at which time, one of the ceramic pastry trays slid from the shelf, striking employee in the chest area.  Employee was injured, slight bruise.  Employee refused medical attention.

Tower Building fire alarm sounded.  Total building evacuation began.  Tempe Fire Department responded, met with College Safety Officer and Maintenance & Operations employee.  Tempe Fire conducted a total assessment of the building, negative results.  Determined the problem was a malfunctioning fire panel.

Synopsis of Incident

While student and her three year old child were inside of the Conference Center, the child intentionally pulled the "fire pull station," causing the alarm to sound and the Tempe Police Department to respond.  The building evacuated, no injuries.
