Synopsis of Incident:

An employee slipped and fell in the break room on the first floor of the Hohokam building. The employee sustained minor injuries to right knee and both hands. No further medical attention was required. An inspection of the area found no cause for the slip.

Synopsis of Incident:

A lightning strike occurred on or near the roof of the Tempe Tower building. This caused an activation of the fire suppressant system on the fifth floor.  The building was safely evacuated. Tempe Fire Department responded. They discovered no fire and provided an "all clear" to return to normal operations.

Synopsis of Incident:

While conducting an early morning assessment, employee leaned over the brick wall for observations.  While leaning over the wall, the employee unintentionally placed both hands on several ant hills, causing injury.  No medical treatment sought or provided.

Known student placed her wallet on the table area, nearest the Testing Center. Known student unintentionally left her wallet. Upon her return to the area (hours later), the wallet was not found, nor was it turned into the Lost & Found.
