Synopsis of Incident
While employee was walking in the hallway, outside of her department
(Instructional Helpdesk), her shoe caught (stuck) in the carpet, causing her to
trip and fall to the floor. Employee suffered injury to her left knee.
Employee sought medical treatment by reporting to Concentra Medical
Center.
