Synopsis of Incident

While walking down the lobby stairs of the Hohokam Building, employee tripped (by missing a stair), causing the employee to fall to her knees, while grabbing the railing.  Employee reported she fell onto both knees, but did not suffer any injury.

Synopsis of Incident

While known community member was utilizing the computer inside of the computer lab, he violated the rules of computer use.  Known community member was escorted from the location.  Known community member was observed to have a strong odor of alcohol on person.

Synopsis of Incident

Reported employees stuck inside of elevator within the Tower Building.  The employees did not suffer any medical emergency and was relieved from the elevator.  Maintenance & Operations notified.  Vendor arrived on location within 30 minutes, correcting the problem.

Synopsis of Incident

The south hall area pipe above the ceiling tiles ruptured, causing contents to leak and fall to the general area of the building.  Maintenance & Operations notified and responded.  The building was closed.  One employee reported medical sickness.  He was treated and released from his personal medical physician.
